Our father, Tony Cuellar, spent two months at Ahwatukee Post Acute (Room 126), and our family is overwhelmed with gratitude. Caring for the elderly and vulnerable is some of the most important work there is, and this team truly lives it every day. Every nurse, aide, physician, and therapist treated him with dignity, patience, and real compassion, even in hard moments. Care-management, social-services, dietary, housekeeping, transportation, and maintenance staff all worked quietly behind the scenes to keep him safe and comfortable. We are deeply, sincerely grateful to every person who cared for him with such humanity and heart. Five stars, without hesitation.

This place is like a prison. I’m writing this while in immense pain and seeing red so please bare with me. After waiting for a few days for insurance to approve my transfer to Ahwatukee acute, it finally came through. I was relieved I could continue my rehab after breaking my tib, fib and ankle. They were even going to let me use my insulin pump which I was very excited about because I manage my diabetes very well and didn’t want someone messing it up. Let’s just say PTSD from a facility I went to for pt a few years back. I was given my meds at the hospital about 3pm. I have many health issues so I take a lot of meds. Transport picked me up around 5pm and I arrived to Ahwatukee Acute at close to 6. First impression was that this facility was clean and the few staff I had met were very nice. My anxiety had started to calm. After check in, I asked for my pain meds to which I was told the paperwork had not yet been processed so my meds were not currently in possession. I was due for all of my meds and none were available. After spending all week getting meds dialed in, there were no longer any to continue my treatment. My pain finally got so intense that I passed out and woke up in pools of sweat. No one wants to take responsibility for this. I was given 3 different stories as to where my pain meds were, why the hadn’t been filled and on and on. Fast forward to 4am. The supervisor finally came in to speak with me and said she was unaware of the entire situation! Let that sink in. THE SUPERVISOR CLAIMED TO HAVE NO IDEA THAT INHAD BEEN SUFFERING BASICALLY SINCE ARRIVAL. I will say that she was able to grab my same pain meds from another cart. Also mind blowing. No way it was just that easy because I had been asking for all my meds without any luck at all. It was like I was speaking a foreign language. I hit the call light and that wasn’t acknowledged in over 30min. Then they seemed to be fed up with me so no one even came to see why I was hitting the light. I can’t have any weight on my leg for 6 weeks per my surgeon and I have had to get myself on and off the toilet with awkwardly placed grab bars, in and out of a wheelchair on my own with no supervision. Just seems like everyone who is here at night is here to play grab ass. This is no care facility. Imagine if I couldn’t advocate for myself like many of the patients here now. Scary. I am giving this facility a chance to redeem themselves but my expectations are very low. I will be getting my case management team involved and will even be placing a call to my attorney. No one should be ignored and treated like I have at this facility. Will update when I’m discharged or if I leave. To be continued…

I am beyond disappointed with my experience at Ahwatukee Acute. This place is horrible and, in my opinion, more concerned with getting paid than providing quality care and addressing legitimate concerns. I have only been to this facility about 4 or 5 times, but the way I was treated during my last visit was unacceptable. When I walked in and asked to speak with whoever was in charge, the way the front desk staff looked at me immediately made me feel like I was not welcome. Then Ashley walked up, and I heard the front desk saying, “It’s so-and-so’s daughter.” After everything that has happened, the way I was treated and made to feel will not be forgotten. I went to the facility to file a complaint about Ashley after she called both my dad and me liars. The way I was spoken to was disrespectful and unprofessional. Instead of taking my concerns seriously and sitting down with me to discuss what happened, it felt like the focus was on getting my dad out as quickly as possible. Less than 24 hours later, my dad was moved, and no one from the facility even called to notify me. I had to find out after the fact. My dad is blind, missing a leg, and uses a wheelchair. As his family, I should have been informed about such a major change. When I asked about phone recordings or documentation of conversations, I was looking for transparency and answers. In healthcare, communication and proper documentation are extremely important. Jose made it seem like he genuinely cared and wanted to help, but after everything that happened, I’m left feeling like I was only being told what I needed to hear so I would leave and stop asking questions. I will be taking my concerns further through the proper channels, including addressing them with the insurance company. I do not feel comfortable only speaking with the people currently working there, especially after Ashley stated she speaks with someone named Jackie from insurance, and I have no idea who that is or what role they have. I work in healthcare, and I know patients deserve dignity, respect, compassion, and proper communication. Families trust facilities like this with their loved ones, and that trust should never be taken lightly. I will continue advocating for my dad because he deserves better. No family should feel ignored, dismissed, or left without answers when it comes to the care of someone they love.

Communication with this place is horrible, not clean and hearing the staff talk about patient information is sad. No call backs or documentation done when needed.

A+++ Excellent Care!. Everyone here cares and the patients are cared for by very personal Nurses. My mom was here for many months and I had the privilege of meeting her nurses. The admins staff if above Excellent!. The physical therapy is 5 star!. You can trust this facility 🙏

My experience at Awatukee Post acute was great from the Start. Nikki actually came to see me in the hospital before I came here. I had lost hope in everything. Felt like I didn’t have a chance of healing or ever getting better. Nikki gave me hope. JR welcomed me when I walked through the doors and I felt calmer almost instantly. JR was extremely helpful and kind and put a smile on my face. Mysti in my opinion is the most valuable employee that exists in this building. She helped me do my intake, fixed my med orders when I first arrived. She goes above and beyond for anything I need and everyone I’ve seen her interact with. She stays late to make sure things are taken care of. Doesn’t get a chance to eat lunch, because shes advocating so much for everyone here. She deserves a raise for how hard she works. I’m surprised she’s not running this place! She is the most knowledgeable and compassionate person and assistant director of nursing I’ve ever met. She’s a diamond in the rough! The moment I met her I knew I was in great hands. She even acts as a patient advocate and will consult with other members of your care team. She made sure my meals were correct and didnt have anything I’m allergic to in them. She made sure to connect me to other people like Jose who is the director of social services and also a solution driven person with so much compassion for the residents here. He has a heart of gold and treats you like a person not a number. Ashley the case manager is also very solution driven,with a heart of gold that goes above and beyond for people. She works hard on your case and truly cares. Greer who helps with Altechs is amazing and knows her stuff! Heather pain management (who also deserves a raise) helped me so much, making sure my pain is managed and under control. Q.Bell Director of Operations she goes above and beyond for this entire facility. She’s a director that wears so many hats. I’ve seen her help wherever help is needed. She helps pass out trays. She brings fresh flowers for the residents, She always has a smile on her face. Q will take time out of her busy schedule if you just need to talk. She helps get anything you need help with or points you to the person who can. She treats everyone like family! She definitely deserves a raise! She’s so good to her staff and the residents.💖 Neil O.charge nurse and Freddy A Unit manager have helped so much with my medical needs, making sure I’m getting to my follow ups with all the tests I need to bring.They deserve a huge raise for all that they do! So does Alba in medical records! Jason D. is the best Nurse I’ve ever met period. He goes above and beyond for every patient I’ve ever seen him interact with. David who does medical transport gets you to all your appointments on time! Kristie the receptionist let’s you know if you have packages, and is always friendly! Jayla LVN ( Extremely kind. sweet and meds always on the dot 💗) She’s on top of her med passes and never misses a beat. Nick night nurse, Iffy night nurse and Tinese and Fereday day nurse’s always have your meds correct and on time with a smile. They go above and beyond! Debbie R. Housekeeping who always makes sure the rooms are sparkling ✨️ she deserves a raise. The Activities director Addy and assistant directors Mona and Donnovan are amazing! They have fun activities that everyone can play whether your in a chair or using a walker, can’t turn your head etc. They have bingo like most places but 3 different kinds. They have nail days and grooming days, which boosts everyone’s self esteem. Painting and perfume making and flower days and bowling and like 20 more activities that are all unique and fun! They are also very kind. They bring so much joy to the residents and deserve a raise! Christine, Josie, Mackayla,Jessica and Johanna & Malachi are the best CNAs I have ever met! They go above and beyond and deserve a raise! If your looking for a post acute for yourself or your loved one, look no further. Awatukee Post Acute is where the healing begins and life gets better! This place is the biggest front, they will pretend they are attentive when you are visiting your loved one but when you are gone they completely disregard care, the patients family shouldn’t have to be the one to ask to provide simple care such as applying lotion or even assisting in changing clothes (mind you he has been in the same clothes for the past times I visited), not being included in meetings or informed is another concern that I brought to their attention and still continue to disregard and neglect the patient and family, I work in healthcare and this place knows the standard and are not holding THEMSELVES accountable for the patients sake. I will be filing a report and providing it to the case worker, director and filing a report with the bureau as I have evidence of this neglect, it is starting to make sense why this location has changed it’s name numerous times. Doctor at this location does his patient visits early in the morning when patients are sleeping to avoid changes in care plan and also avoid any recommendations by the family, making it impossible to speak with him later on in the afternoon, makes sense why this location changes their name often.

My sister came to this facility for all sorts of medical problems. The nursing team was great and she is now at home with family. We feel blessed that their team is on top of their game.

I had a really great experience with Ahwatukee Post Acute. From the very beginning, the admissions team was incredibly easy to work with and made the entire process feel smooth and stress free. The facility itself was welcoming and had such a positive atmosphere. Every staff member we interacted with was friendly, helpful, and genuinely kind, which made a big difference. You can truly tell they care about the people they serve, and I would absolutely recommend Ahwatukee Post Acute to anyone looking for compassionate, quality care.

Everything I’m being released from our Tukey postacute rehab. It’s been very good. There’s a social worker here. Her name is Ashley. She’s really good to me and she was able to get me discharge properly and on schedule. She was very very good to me. Thank you.

I live my CNA Lubby. She made me laugh on my first day there. She would say Whats up Ms. B?!. The nurses and PT/OT are awesome. Especially Mrs. Star! What I don’t appreciate is that they refused to let me go home. They came up with every excuse in the book. Their food is…y’all can you say salty!!

Ahwatukee Post Acute is a great place . Very caring attitude towards staff. The Nurses and CNA’s go out of their way to make patients feel better. The PT & OT is excellent . SW & Admin people are friendly and helpful. A great place for Rehab !

If No Star was possible, that would be my rating. They were criminally negligent of my relative while he was in respiratory distress, and, after he was released to a new facility, I, who have Medical Power of Attorney, was made to wait over an hour for his personal property which, as at other places, should have taken no longer than 15 minutes. The front entrance smelled strongly of urine, and they don’t pay attention to their residents riding around in electric chairs almost naked….. KEEP YOUR LOVED ONES AWAY FROM THIS PLACE!!!!!!!
